Exosomes explained...

Exosomes are the by-product of cells. In simple terms, they are the bud-like component created when two cells fuse together. They are often described as microscopic bubbles or in the scientific realm as nano extracellular vesicles.

Eventually, the bud-like components break away from the fused cells, but these tiny molecules don’t fizzle away. They have an enormous role to play in skin rejuvenation.

An interesting fact about Exosomes is that although they’re not cells—which means they don’t have the ability to replicate, they are however important for tissue healing, regeneration and repair. Ok, you may be wondering, how is this possible?

Exosomes contain biomolecules including proteins, growth factors, lipids and nucleic acids such as messenger-RNA, (mRNA) and micro-RNA (miRNA). They act as messengers from stem cells to other cells. One way to imagine Exosomes at work is to think of them as little envelopes of regeneration from cell to cell.

So what this boils down to is that Exosomes have enormous benefits for the skin! They have been shown to:

  • Be an effective form of skin rejuvenation for the face, neck and hands.
  • Drastically diminish the appearance of upper lip and smile lines, crow’s feet, and neck rings.
  • Reduce the appearance of age spots and acne scars including skin depressions.

While Exosomes have created a real buzz in skin rejuvenation therapy, they are not a passing trend. Exosomes are revolutionising skin rejuvenation.

Now that you know about Exosomes, would you like to know where they come from?  

Exosomes are connected to stem cells and are retrieved from Mesenchymal Stem Cells (MSCs). Yes, that does sound very scientific, but it’s all done above-board. They are ethically harvested from consenting donors and are extracted and synthetically processed in a laboratory to strict protocols without any dilemmas or risk of corruption or cell mutation.

While the regenerative techniques of Exosome Therapy, Stem Cell Therapy and to some degree Platelet Rich Plasma Therapy are often grouped together, the truth is, they are all rather different. Let’s look at each of these.

Regenerative Medicine – 3 different therapies

Stem Cell Therapy

Stem cell therapy for skin rejuvenation utilises human stem cells to help fill in tissue and create more collagen for better looking skin. 

The reason stem cells are used for this therapy is that they have the unique ability to transform from one cell type to another. They can replace and repair damaged cells. And as they grow and divide, they heal and rejuvenate.

There is an abundance of stem cells in fat tissue therefore the process often involves collecting stem cells from fat deposits. This is done via liposuction, in which case a local anesthetic would be required.

Stem cells can also be extracted from blood. Once the extraction is complete, the stem cells need to be isolated. This may be done offsite at a lab or via a centrifuge. The stem cells are mixed with a solution of growth hormones before they are injected into the skin.

Multiple injections may be required to various areas for example the lips, chin or cheeks.  Some skin care specialists may be able to sculpt or contour for a specific finish. But this is a slow process and skin rejuvenation takes time.

While this therapy is certainly not surgery, if liposuction is involved, it may be considered a tad intrusive

The idea of stem cell therapy may sound good in theory but there is a school of thought that believes otherwise. Stem cell therapy is nothing more than a process of depositing fat to smooth out wrinkles by adding volume.

Platelet-Rich Plasma (PRP) Therapy

Platelet Rich Plasma or PRP is an effective skin rejuvenation technique that has received wide media coverage with celebrities undergoing the treatment for a youthful complexion. When combined with Dermapen it’s commonly referred to as the ‘vampire facial’ because it involves drawing blood and then smothering the skin with it. It may sound like something from a horror movie but those who undergo the procedure say they can see the benefits on their skin.  

Curious to know more about how PRP works? The process involves drawing blood from the patient, usually from the arm—just like a nurse would do for a blood test. This step alone makes it not the best approach for the faint-hearted. (But don’t worry we have a solution for you soon)!

Once drawn, a centrifuge spins the blood at high speeds to separate the PRP from the rest of the blood which is not needed and discarded. The PRP is highly concentrated, full of nutrients and growth factor proteins which are known to assist the skin to heal, particularly for wound healing.

When this skin rejuvenation trend was introduced, it was injected directly into skin of patients but it’s now common practice to do PRP therapy as part of a microneedling rejuvenation treatment. By combining this therapy with microneedling, the PRP is massaged into the skin immediately after needling. The micro-channels allow the PRP to be delivered into the deep layers for rejuvenation and healing.

While PRP with microneedling gets results for patients, it’s a messy, time-consuming and expensive process. If injected into the skin, it can also cause swelling, redness and pain at the injection sites. Some patients may be deemed ‘compromised’ for PRP which is considered an autologous procedure, while other patients may not want blood drawn.
